Идея обучить начинающих хирургов сосудистому шву замечательная, но обучающий человек должен в совершенстве владеть сосудистой техникой и знаниями сосудистой хирургии. В настоящем ролике допущено очень много ошибок. Я назову только грубые ошибки, которые совершать ни в коем случае нельзя. 1. протез , в силу его ригидности повдоль не рассекают, а изогнутыми ножницами вырезают овальное отверстие (это позволяет исключить турбулентность кровотока); 2. первые два шва, которые накладывают на "носочек" и "пяточку" сшиваемых сосудов должны быть П-образными (для того чтобы сшиваемые сосуды соприкасалась интимой); 3. сосудистый шов начинают с наложения двух держалочных швов (в данном случае "носочек", "пяточка"); 4. сосудистый шов, также как и любой другой шов, шьют на себя, а не от себя; 5. расстояние между вколами должно быть одинаковое; для того чтобы стяжки ложились ровно, хирург пинцетом (левой руки) удерживает нить в основании при затягивании. Шов, который я здесь увидел применяют в урологии при формировании цистоуротеростомии. Удачи!
